In the first quarter of 2026, Autumn Lake Healthcare Post-Acute Care Center was ranked the second largest nursing home in Baltimore City County, data from the Centers for Medicare and Medicaid Services (CMS) shows.

Records indicate the center provided 225 beds, exceeding Baltimore City County’s average by 92 beds for the period. On average, 196.6 residents received care at the facility that quarter.

The CMS gave Autumn Lake Healthcare Post-Acute Care Center an overall rating of 4 out of 5, which is above the Maryland nursing home average of 3.1.

Keswick Multi-Care Center led the county with 242 beds, making it the largest nursing home, according to quarterly data. Levindale Hebrew Ger Center & HSP held the third position by size with 210 available beds.

As of Q1 2026, Autumn Lake Healthcare Post-Acute Care Center reported no fines or penalties.

There were 21 for-profit nursing homes operating in Baltimore City County, Maryland, for that period.

U.S. Census Bureau estimates indicate that the older adult population continues to rise, while younger age groups are shrinking. From 2020 to 2024, the 65-and-older demographic rose by 13%, but the population under 18 dropped by 1.7%. Over the last century, the United States’ older adult population has surged from 4.9 million in 1920 to more than 61 million by 2024.

According to a 2021 study, close to half of this age group—about 28 million Americans—are likely to require long-term services and supports. The study also estimates that over one-third may ultimately need nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
